Horizons is a 76-bed Assisted Living Facility designed to meet the needs of advancing age. Our residence is situated in Canandaigua, New York in the beautiful Finger Lakes region, 25 miles southeast of Rochester. Horizons is the only facility in our area also licensed to participate in the Assisted Living Program (ALP) which provides a higher level of care as an alternative to placement in a nursing home. Medicaid is accepted for this program.
The needs of individuals can change significantly over time. Horizons provides the services necessary to meet these unscheduled changing needs. We work closely with residents, their families, physicians and any community agency that might be involved with that resident. An individualized plan of care is developed to meet the needs of each resident.
